Programs and Levels of Care

What types of addiction treatment programs does Sobrius offer?

Sobrius offers a full range of addiction treatment in Virginia, from live-in residential care to ongoing support after you return home.

Our admissions team will help you find the right starting point. Call 888.596.6514.

What is the difference between the 3.5 and 2.5 levels of care?

The numbers come from the American Society of Addiction Medicine (ASAM) levels of care. Level 3.5 is residential treatment: you live at Sobrius full time with 24-hour support, and an on-site chef prepares your meals. Level 2.5 is partial hospitalization (PHP): you attend a full treatment day but have more independence, such as access to your phone after classes, passes to visit loved ones, the option to work in the evening, and the choice to live in Sobrius PHP housing or commute from home. Many clients start in residential care and step down to PHP.

Does Sobrius have separate programs for men and women?

Yes. Sobrius offers a men's rehab program in Galax and a women's residential program at our Women's Recovery Center in Bassett. Gender-specific treatment gives clients a safe space to talk openly about the challenges that tend to affect men and women differently, such as trauma, relationships, and family responsibilities.

Does Sobrius offer detox?

No, Sobrius does not provide detox on site. We partner with reputable detox centers so you can move directly into treatment at Sobrius once detox is complete. Our admissions team can help coordinate that transition.

Do I have to stay for the full length of the program?

No. Treatment at Sobrius is voluntary, although a court order may set its own requirements. There is no fixed number of days to complete a program. Your length of stay is based on your treatment plan and your progress, which you review regularly with your treatment team.

Can I commute to Sobrius for treatment?

Yes. Clients in our partial hospitalization program (Level 2.5) can commute to Sobrius each day instead of living in our PHP housing. Residential (Level 3.5) clients live on site during treatment.

Will Sobrius work with my probation officer or Recovery Court?

Yes. Once you sign a release of information (ROI) for your probation officer or Recovery Court, Sobrius can communicate with them directly and keep them informed of your participation in treatment.

What makes Sobrius different from other treatment programs?

Sobrius treats the whole person, not just the addiction, in small, home-like settings in the Blue Ridge Mountains of Virginia. What clients notice most:

  • Daily one-on-one therapy with your individual therapist, plus group therapy every day.
  • Holistic and evidence-based care together, including CBT, motivational interviewing, relapse prevention, and Trauma Relaxation Therapy.
  • Gender-specific residential programs for men in Galax and women in Bassett.
  • A full continuum of care, from residential to PHP, sober living, aftercare, and a lifelong alumni community.
  • Home-cooked meals from an on-site chef.
  • Experience with the justice system, including coordination with probation officers and Recovery Courts.
  • Medicaid accepted, along with several major insurance plans.

Sobrius is led by CEO Nelson Smith, a former Virginia Behavioral Health Commissioner.

Addiction and Therapy

What types of addiction does Sobrius treat?

Sobrius treats all common substance use disorders, including addiction to alcohol, opioids, fentanyl, heroin, cocaine, methamphetamine, marijuana, benzodiazepines such as Xanax, and other prescription drugs. We also treat co-occurring mental health conditions such as anxiety and depression alongside addiction. See all of our substance abuse treatment programs.

What types of therapy does Sobrius use?

Sobrius combines widely used, evidence-based therapies with holistic care. Options include cognitive-behavioral therapy (CBT), group therapy, motivational interviewing, relapse prevention therapy, holistic therapy, and Trauma Relaxation Therapy. Most clients benefit from more than one approach. Learn more about our addiction therapy programs.

Will I get one-on-one therapy?

Yes. Every Sobrius client is assigned an individual therapist and has access to them daily. Individual sessions last about one hour and are part of every level of care, alongside daily group therapy.

Who decides what type of therapy I need?

You and your treatment team decide together. When you arrive, our clinicians complete a full assessment that looks at the substances involved, your history, your mental and physical health, and your goals. Your team then recommends therapies that fit your situation, and you choose the options that work best for you. Your plan is reviewed and adjusted as you progress.

Is there a psychologist or licensed clinician on site?

Yes, Sobrius employs qualified clinicians at every location who provide individual and group therapy. [CONFIRM: list credentials on staff, e.g. LPC, LCSW, psychologist, psychiatric provider]

Medication-Assisted Treatment (MAT)

What is medication-assisted treatment (MAT)?

Medication-assisted treatment (MAT) combines FDA-approved medication with therapy and counseling. The medication helps reduce cravings or block the effects of a substance, while therapy addresses the thoughts and behaviors behind the addiction. Together, they can improve your chances of long-term recovery.

Is MAT used for every type of addiction?

No. FDA-approved medications currently exist for opioid use disorder and alcohol use disorder, so MAT is used for those two types of addiction. Other addictions are treated with therapy and holistic care.

What type of MAT does Sobrius offer?

Sobrius offers MAT with Vivitrol (naltrexone), a monthly injection that blocks the effects of opioids and reduces cravings for alcohol. Because Vivitrol is not addictive, it can help you avoid relapse once you are no longer using opioids or alcohol.

Can I cook for myself?

It depends on your level of care. Residential (Level 3.5) clients do not cook; the on-site chef prepares all meals. Partial hospitalization (Level 2.5) clients in Sobrius PHP housing can buy and cook their own food, except at our Lynchburg PHP, which has an on-site chef.
